Introduction for Case Managers
The inspiration for AECN comes from a multi-generational collection of desires to raise home healthcare standards and treat our community’s aging population with dignity and compassion.
Because of your responsibilities, you, the case manager, are as important a client to us as the patients you serve or the residents in our care.
Our service is designed to align with our goals tightly, setting up the next generation of home healthcare professionals for higher standards and practice.
Our Placement Services
-
Multiple Options
At AECN, we understand the importance of providing options to families looking to place their loved ones. AECN agents remain closely linked with board and care communities to ensure the best options are provided based on the patient/client’s personality, preferences, and essential care needs. Our goal is for these efforts to result in a successful placement, meaning your patient found the right place for them and will not need to return.
-
Experienced Agents
The AECN team comprises experienced, licensed, and certified professionals who have dedicated themselves to their community by giving back to the generations before them. Our agents are required to train in the board and care environment. We believe this experience equips them with a solid foundation of knowledge to make responsible recommendations and an ability to communicate effectively with other health professionals.
-
Professional Coordination
One of the most critical aspects of AECN’s placement service is to streamline the discharge process on behalf of the patient/client, their families, and you as the case manager. When all criteria for placement are met, our team will know exactly what to expect. From DME orders to patient transport, all preparations will ensure your patient's safe and smooth transition from your facility to their new home.
